Giveaway Information 

We are giving away Free Braces to one lucky Las Vegas Dad! Entering the giveway is easy! All you have to do is talk to Dad about his smile and fill out our online entry form. Submissions are due by June 19th before 12 pm, noon PST. One entry per Dad. Winner will be chosen randomly. Dad must live in Las Vegas for treatment. NO PURCHASE NECESSARY TO ENTER OR WIN. 

The giveaway Free Braces For Dad is only open to Dad residents of the United States residing in the city of Las Vegas, Nevada who are 18 years of age and older who enter via online submission form. The giveaway consists of a random chance of winning.

***Any necessary dental work required prior to starting Braces is NOT included and will need to be completed by Dad’s dentist.***

The giveaway promotion begins when the announcement of the online submission form is given. The end of the promotion is on Friday, June 19th, 2020 at 12pm, noon PST.

  1. Fill out the complete online form, including the images, as requested. Be sure to provide all required information accurately.

Entry Rules

Enter a Las Vegas Dad to win Free Braces from Smile Squad. ***Any necessary dental work required prior to starting Braces is NOT included and will need to be completed by Dad’s dentist.***  All submissions must be entered to our online submission page. Contest ends on Friday, June 19th, 2020 at 12 noon PST. All submissions must be fully filled to be entered, including images requested by the online entry form. It is suggested to nominate a Dad, however, Dad, he, himself, may submit on his behalf to online giveaway. Giveaway winner will be contacted on Friday June 19th, afternoon. Winner will be announced to the public on social media, no later than June 22nd, 2020. Winner will be chosen at random. Only one entry is necessary per Dad nominee. Only one entry will be registered for any duplicate entries.  No purchase is necessary to enter this giveaway. Dad must live in Las Vegas for treatment. Treatment must be started by December 1st, 2020 and no later. Giveaway prize void after December 1st, 2020.

PARTICIPATION GUIDELINES:

  • Must be a resident of Las Vegas
  • Must follow the treatment plan and agree to show up to all appointments on time
  • Must agree to see a dentist every 6 months for a cleaning

TERMS AND CONDITIONS

  • By submitting an entry to this contest, the participant acknowledges and agrees to all of these Terms & Conditions.
  • Winner of the contest will receive a certificate for FREE ORTHODONTIC TREATMENT including a full comprehensive course of orthodontic care from Smile Squad Kids Dentistry & Braces, an estimated value of $5,500. This certificate is all-inclusive, covering the diagnostic materials, all necessary appliances, all active therapy (averaging 2 years), and all necessary retainers and follow-up visits. This offer does NOT include any necessary dental treatment, oral surgery, cleanings, etc. that may be required prior to receiving treatment. Certificate may not be redeemed for cash.
  • Winners will be required to sign a release of liability, W9 Tax Form and photo/video release form upon winning.
  • If selected winner does not respond to either to phone or email contacts made by Smile Squad Kids Dentistry & Braces personnel to inform him of being selected as the winner within 24 hrs, then another winner shall be chosen, with no further entitlement to the initial winner.
  • Winner agrees to document their experience with Smile Squad Kids Density & Braces with photos, videos and interviews.
